The innovative healthcare delivery model that incorporates interdisciplinary care is the Patient-Centered Medical Home (PCMH) model. This model emphasizes on provision of relationship-based care that meets the needs of the whole person. PCMH requires partnering between the families and patients in meeting the patient's culture, unique needs, preferences, and values (Alsalamah, 2021). It recognizes that the patients and family members are important members of the care team and medical home practices ensure that they are informed in the establishment of care plans. Interdisciplinary care delivery is witnessed in PCMH healthcare providers who are connected through blogs, portals, and websites. Collaboratively, the healthcare providers can determine the solutions to the problem and ensure that problems are solved quickly and easily. However, care coordination is not as easy as only a few families can adapt to PCMH. Generally, in healthcare, health disparities are mitigated, healthcare costs are reduced and emergency visits are lowered. Additionally, the new version of PCMH incorporates feedback from physicians, nurses, and other certified experts in care delivery (Alsalamah, 2021).

PCMH improves patient outcomes by improving the quality of care by emphasizing coordination, communication, and team-based care. It improves patient experience as reported by the majority of the patients as they are actively engaged in care provision. The model encourages better management of chronic diseases as all the stakeholders are involved in the delivery of high-quality care. PCMH aligns with federal and state initiatives such as value-based care that reimburses healthcare providers based on the quality outcomes derived (CDC, 2021). The model is associated with the staff's satisfaction with the care providers. An increase in staff satisfaction is associated with high morale and motivation in providing care that yields the best patient outcomes. Additionally, PCMH lowers healthcare costs to the benefit of the patients as it also improves the care provided and reduces the associated costs.
